As in other countries, the traditional doctor-patient relationship in the Japanese healthcare system has often been characterised as being of a paternalistic nature. However, in recent years there has been a gradual shift towards a more participatory-patient model in Japan. With advances in technology, the possibility to use digital technologies to improve patient interactions is growing and is in line with changing attitudes in the medical profession and society within Japan and elsewhere. The implementation of an online patient engagement platform (...) is being considered by the Myotonic Dystrophy Registry of Japan. The aim of this exploratory study was to understand patients’ views and attitudes to using digital tools in patient registries and engagement with medical research in Japan, prior to implementation of the digital platform. We conducted an exploratory, cross-sectional, self-completed questionnaire with a sample of myotonic dystrophy patients attending an Open Day at Osaka University, Japan. Patients were eligible for inclusion if they were 18 years or older, and were diagnosed with MD. A total of 68 patients and family members attended the Open Day and were invited to participate in the survey. Of those, 59 % submitted a completed questionnaire. The survey showed that the majority of patients felt that they were not receiving the information they wanted from their clinicians, which included recent medical research findings and opportunities to participate in clinical trials, and 88 % of patients indicated they would be willing to engage with digital technologies to receive relevant medical information. Patients also expressed an interest in having control over when and how they received this information, as well as being informed of how their data is used and shared with other researchers. Overall, the findings from this study suggest that there is scope to develop a digital platform to engage with patients so that they can receive information about medical care and research opportunities. While this study group is a small, self-selecting population, who suffer from a particular condition, the results suggest that there are interested populations within Japan that would appreciate enhanced communication and interaction with healthcare teams. Knight has shown how the moral growth of medical students involves a spiritual journey. He may, however, present too sanguine a portrayal of the extent to which the medical education environment promotes this moral and spiritual growth. Medical school may indeed be more abusive than supportive. Admitting more women to medical school and teaching more humanities courses, while worthwhile, will not necessarily promote the goals that Knight appropriately advocates.

John Locke's Essay Concerning Human Understanding is among the most important books in philosophy ever written. It is a difficult work dealing with many themes, including the origin of ideas; the extent and limits of human knowledge; the philosophy of perception; and religion and morality. We argue that the robustness analysis of idealized models can have confirmational power. This responds to concerns recently raised in the literature, according to which the robustness analysis of models whose idealizations are not discharged is unable to confirm the causal mechanisms underlying these models, and the robustness analysis of models whose idealizations are discharged is unnecessary. In response, we make clear that, where idealizations sweep out, in a specific way, the space of possibilities— which is sometimes, though not always, (...) the case—they can be holistically discharged. In turn, this can be used to show that the robustness analysis of idealized models can have confirmational force after all. (shrink)

Children who experience well-being are engaging more confidently and positively with their caregiver and peers, which helps them to profit more from available learning opportunities and support current and later life outcomes. The goodness-of-fit theory suggests that children’s well-being might be a result of the interplay between their temperament and the environment. However, there is a lack of studies that examined the association between children’s temperament and well-being in early childhood education and care, and whether this association is affected by (...) ECEC process quality. Using a multilevel random coefficient approach, this study examines the association between toddlers’ temperament and well-being in Norwegian ECEC and investigates whether process quality moderates this association. Results reveal an association between temperament and well-being. Staff-child conflict moderates the association between shyness and well-being, and between activity and well-being. Moreover, high emotional behavioral support moderates the association between activity and well-being. Extra attention should be paid by the staff to these children’s needs. (shrink)

Flower and Josiah OsgoodThe five articles in this special issue of AJP seek to advance our understanding of republican Rome by paying close attention to women in relation to space. Using a range of sources and approaches, contributors find women throughout the city of Rome—on the streets, in the Forum, in houses (some of which were owned by women), and at (...) temples and sacred structures such as the Regia, a building the Romans believed had once been the house of their kings. Contributors consider how women participated in political and legal discussions, accrued and controlled property, and fulfilled ritual obligations deemed vital to the community's well-being, often side by side with men. Access to different kinds of spaces afforded women social status, recognition, and influence within society at large.A central concern of the authors is to show that public life, broadly defined, involved women as well as men. By public life, we mean, in part, activities that occurred in the open, where large numbers might gather, watch, listen, and respond. In his contribution to the issue, Lewis Webb shows how women of very different statuses held religious roles—some as priestesses—roles that required them to move through the city and carry out sacrifices and other rites in a highly visible manner. In her article studying non-elite women's part in politics in the late Republic, Celia E. Schultz looks at various women who traveled in the entourages of male politicians and the attention they drew to themselves. Josiah Osgood, in his discussion of high-ranking women in the same period, shows how women introduced their concerns into politics, through conversations [End Page 1] held in domestic settings but also by protesting publicly and testifying as key witnesses at criminal trials.Public life encompassed activities that were of concern to the community, such as religious rituals performed on behalf of that same community. As Meghan DiLuzio points out in her article on women in the Regia, "visibility is not the only way of thinking about what made a ritual public." The Romans defined a ritual as public (publicus) if it was "paid for by the community and performed on behalf of the people." In her contribution, Harriet I. Flower points out that political discussions and negotiations could happen in houses (Roman politicians did not have separate offices) and then considers examples of such conversations that are attested in houses owned or controlled by women. In Livy's narrative of the so-called Bacchanalian conspiracy of 186 b.c.e. (39.8–19), Flower investigates how a series of pivotal conversations each took place in women's houses. Narrative historians of Rome often focused on warfare, other activities of magistrates, and formal political debate at contiones and in the Senate, spheres in which women's voluntary participation was typically precluded, and this view of what mattered often gets repeated today, at times in general treatments. Closer attention to women, and to the spaces in which they were present and active, opens up a fuller picture of republican culture. In this culture, religion was not a scholarly sub-field but a central part of social and political experience, and political debate and decision-making comprised far more than individual orators standing on the Rostra or in the Curia.This special issue of AJP grew out of a particular interest in space as a focus for historical analysis. We wanted to ask basic questions about where women spent their time, what discussions and decision-making fora they could be a part of, and how they moved through the city. Meanwhile, no answer to these questions can omit religious activities, which feature heavily in most of the following articles. Cascardi's fascinating "Skepticism and Deconstruction," I am led to ask the question that "entitles" this response.1 The answer I want to give is "yes," but Cascardi has made the task more difficult than I would have liked. In brief, he has dissociated deconstruction from all philosophical pursuits, including skepticism, which it (...) superficially resembles. I shall proceed, first, by reasserting deconstruction 's skeptical credentials and, then, by showing how these credentials enable the deconstructionist to play an interesting language-game. I Cascardi's thesis is most vividly stated in the following passage: But the deconstructionist, wrongly thought of as a skeptic, has no idea of what an epistemology counter to his uncertainties and indeterminacies would look like. This is because he is out to question the very axis ofwhich skepticism and epistemology are the opposite poles. Because his questioning shows up as a denial of the principle of non-contradiction, one is likely to think that deconstruction is a form of irrationalism. (p. 7) For all its forcefulness, this statement suggests that Cascardi may have fallen prey to a Cartesian ruse involving the meaning of "skepticism." A coherent history of skepticism can be told that extends from Sextus Empiricus's Outlines ofPyrrhonism (c. 200 AD) to Pierre Bayle's Historical and Critical Dictionary (c. 1700) without ever mentioning Descartes. The theme 104 Steven fuller105 of this history would be that no claims to knowledge are certifiable. In a more modern vein, we would say that no proposition has a determinate truth-value. The skeptics in our history would be presented as moral philosophers instructing their readers on how to live in a world in which they can never tell the true from the false — a world in which doubt is the natural attitude. The paraphrase is crucial because virtually all philosophers — most notably Husserl — have regarded doubt as unnatural, a psychic tension that must somehow be resolved. And just as hunger motivates the drive for food, it has usually been assumed that doubt motivates the drive for method. Whereas food sustains the physical needs of the sentient being as it reduces hunger, so too method sustains the cognitive needs of the rational being as it reduces doubt. Our skeptics have perennially noted that to think of all knowledge claims as certifiably true or false would be to think that nature answers us directly upon being addressed. In less metaphorical terms, we would be speaking a language whose predicates pick out natural kinds. The "dogmatists," as Sextus and Bayle liked to call their opponents (we would now call them "foundationalists"), could then easily conceive of how nature appears to us as nature's response to our utterances. To drive home the dogmatists' intentionalist stance toward nature, instead of "how nature appears to us," we might say "how nature represents itselfto us." In the case of truths, nature represents itself transparently, while in the case of falsehoods, representation has gone awry: either we misspoke or nature deceived us. To put the blame on ourselves is to call for a revision in method; to put the blame on nature is to suspect that an evil designer, a Cartesian demon, is behind the faulty appearances. The point, of course, is that one needs to be a dogmatist in order to have Descartes's more demonic thoughts. The classical skeptic, who held the language-world link to be purely conventional, used the uncertifiability of knowledge claims as an argument for denying any design whatsoever to nature — either for good or for ill. Descartes muddled things, however, when he interpreted the skeptical stance as claiming that all knowledge claims may turn out to be false. From the First Meditation, it is never made clear why we should worry about the Cartesian demon (and hence send him into cognitive exile as quickly as possible). Presumably, the concern arises from the possibility of discovering that the appearances are indeed illusory — that is, certifiablyfalse. Science and Nature magazines celebrated the event by establishing interdisciplinary websites where readers can access essays on aspects of genome mapping and directly link to databases to search for more information on the topics (IV. Since 1994, the National Information Resource on Ethics and Human Genetics (NIREHG), a special project of the National Reference Center for Bioethics Literature (NRC), has been providing such an integrated information service on ethics and genetics. Designed to enable a wide range of users to go from annotated bibliographies (Scope Notes) and brief essays (called "enotes") to database searching, NIREHG fosters an interdisciplinary approach to research on ethics and genetics. The 301 letters, most of which have never before been published, have been collected from archives, public and private, in Britain, the United States of America, Switzerland, France, Japan, and elsewhere, as well as from the major collections of Bentham Papers at University College London Library and the British Library.In mid-1824 Bentham was still (...) preoccupied with the Greek struggle for independence against Turkey, though his active involvement waned as he became disenchanted with the behaviour of the deputies sent to London by the Greek National Assembly. His international reputation was reflected in his continuing contact with Simón Bolívar and Bernardino Rivadavia in South America, and with John Quincy Adams, John Neal, Henry Wheaton, and others in the United States, and his forging of new contacts in Guatemala, India, and Egypt. In the autumn of 1825 he visited France, where he stayed with Jean Baptiste Say and La Fayette, and was fêted by the French liberals.Bentham made considerable progress drafting material for his pannomion, or complete code of laws, and in particular for his Constitutional and Procedure Codes, while John Stuart Mill edited the massive Rationale of Judicial Evidence. The term ‘consciousness’ is usually reserved only for the focus of attention. This restriction empties the phenomenology of consciousness of some of its richness. Rather than conceiving of consciousness as one-dimensional, researchers should consider that consciousness has a three-dimensional organization. Conscious presentations are structured in a focus, context and margin pattern. Inclusion of these other dimensions of consciousness as consciousness is important for an adequate relation between scientific method and phenomenology. The problem becomes especially acute when transformations in consciousness -- (...) attentional and temporal continuity -- are considered. Using Aron Gurwitsch's work, this paper presents an alternative to Galen Strawson's view of consciousness and the self, as an example of the usefulness of this fuller conception of consciousness. I argue here that there is significant attentional and temporal continuity in consciousness, and that this continuity provides for a sense of the self as a distinct, but continuous experience. Most moral dilemmas in medicine are analysed using the four principles with some consideration of consequentialism but these frameworks have limitations. It is not always clear how to judge which consequences are best. When principles conflict it is not always easy to decide which should dominate. They also do not take account of the importance of the emotional element of human experience. Virtue ethics is a framework that focuses on the character of the moral agent rather than the rightness of (...) an action. In considering the relationships, emotional sensitivities, and motivations that are unique to human society it provides a fuller ethical analysis and encourages more flexible and creative solutions than principlism or consequentialism alone. Two different moral dilemmas are analysed using virtue ethics in order to illustrate how it can enhance our approach to ethics in medicine. (shrink)

Anne Conway: A Woman Philosopher, by Sarah Hutton; 280 pp. Cambridge: Cambridge University Press, 2004. $75.00.Recent work on women philosophers has, in general, approached the topic from two vantage points: on the one hand, a number of anthologies have (...) been published, wherein commentators present a small portion of the woman thinker's work, often without the background necessary for a fuller understanding. On the other hand, some few women philosophers have been the subject of full-length works, but these have tended, by and large, to be women thinkers of the twentieth century, with whose work we are already familiar. Two recent books, one by Jacqueline Broad and the other by Sarah Hutton, go a long way toward improving the situation.Work by Eileen O'Neill, Sarah Hutton and Margaret Atherton, among others, has helped familiarize us with the notion that the seventeenth century—that era so commonly associated with Descartes and the rise of modern philosophy—was also a time of rich theorizing by women philosophers. Jacqueline Broad's book possesses many virtues: it has the finely-tuned style of the careful historian, while at the same time not failing to demonstrate acquaintance with a range of contemporary feminist commentary. Part of the problem in addressing women thinkers of the past is another version of what we are now tempted to call the essentialism problem: does it make sense to think that there is a style that might be demarcated as "feminine," or that might, in some way, pertain uniquely to women theorists? These are difficult issues, but a partial response to the question is to try to remember that it is not for no reason that women thinkers have, indeed, been left out of the canon. As Broad herself says, women philosophers "are now mere footnotes to the standard historical-intellectual accounts of the early modern period" (p. 1).Broad does a superb job of navigating between the need to set out the plain tenets of the thinker's philosophy, and the desire to try to tie it into these larger questions. In six chapters, she addresses the work of Elizabeth of Bohemia, [End Page 200] Margaret Cavendish, Anne Conway, Mary Astell, Damaris Masham and Catharine Trotter Cockburn The chapters on Elizabeth, Margaret Cavendish and Mary Astell are particularly strong—here part of the difficulty is that we do have familiar commentaries addressing the work of these women, and it is hard to try to see them in a new light. We are accustomed, for example, to think of Elizabeth merely as Descartes's correspondent. As is the case with information that is currently coming to bear on the question of Harriet Taylor Mill's input into the work of John Stuart Mill, it is difficult for us to see Elizabeth as a thinker in her own right, or, indeed, to give her the credit that Descartes himself gave her; as Broad notes, he had described her as "the only person I have so far found who has completely understood all my previous works" (p. 13). Margaret Cavendish's reputation has suffered from the somewhat scattered approach of her writings, and Broad cites her "reputation for eccentricity" (p. 38). Nevertheless, the sheer volume of her work and its allusion to the problems addressed by the Cambridge Platonists (areas also worked on by Anne Conway) demands that she be taken far more seriously than she has in the past. Astell has become known largely for her more overtly feminist works, such as A Serious Proposal to the Ladies, but she was also the author of several other pieces addressing issues in metaphysics, and again the amount of her work and its citation during her own time would militate against the notion that it should not receive thorough examination.Broad does a wonderful job of setting out the main philosophical questions found in these theorists' works, and she excels at pitting their views against those of the various male philosophers whose work—according to... After a married woman with four children submitted a prescription for the morning-after pill, the pharmacist, Neil Noesen, not only refused to fill it, but also refused to transfer the prescription to another pharmacist or to return the prescription (...) to the customer. As more such incidents occurred, many states "... decided to consider and enact laws setting the bounds of pharmacists' and other health care workers' professional obligations" (III, Grady 2006, p. 327). Discussions of objector legislation, also referred to as "conscience clauses," "refusal clauses," and "abandonment laws" (III, Appel 2005, p. 279), are not limited to professional ethics, but also draw from philosophical, theological, and legal perspectives. The purpose of this Scope Note is to present a wide variety of viewpoints on the health provider's right to conscience.More than 40 years ago the development of "The Pill" as the first reliable method of birth control not only ushered in a feminist revolution, but also provided a new focus for concerns of conscience for those who were part of the anti-abortion movement based on religious belief in the sanctity of life. Similarly, in the past ten years, worldwide, and seven years (1999) since the emergency contraception "morning after" pill first became available as a prescription item, there has been an upsurge in the number of medical personnel who refuse to prescribe or dispense it on grounds of personal conscience, whether for religious reasons or not. Their actions bring into play issues of power and control for health care personnel and for patients—in this case women, which also raises women's rights issues. Ironically, studies in France, Sweden, and the United Kingdom have shown that emergency contraception does not reduce the abortion rate—it is too infrequently used (II, Glasier 2006).It is important to underline the difference between the "morning-after" pill or "Plan B," which is made up of two progestin pills containing levonorgestrel (a synthetic derivative of the female hormone progesterone), and RU-486 (Mifiprex [End Page 379] or mifipristone with misoprostol). Plan B, if taken within 72 hours post-coitus prevents implantation, and therefore pregnancy, by suppressing the output of luteinizing hormone, the hormone that triggers the ovulation process. Scientists have been unable to determine whether this action could destroy already fertilized eggs, but even if it does, it uses the same mechanism as occurs with the birth-control pill, that was developed some 45 years ago. By contrast, RU-486 acts up to 49 days after implantation by blocking the action of progesterone in order to terminate the pregnancy and as such is an abortifacient (II, US FDA 1).On 24 August 2006, the U.S. Food and Drug Administration announced approval of the Plan B pill for over-the-counter (OTC ) sales (II, US FDA 2). In this follow up to The Eudemian Ethics of Aristotle, Peter L. P. Simpson centers his attention on the basics of Aristotelian moral doctrine as found in the Great Ethics: the definition of happiness, the nature and kind of the virtues, pleasure, and friendship. This work's authenticity is disputed, but Simpson argues that all the evidence favors it. Unlike the Nicomachean and Eudemian Ethics, Aristotle wrote the Great Ethics for a popular audience. It gives us insight less into Aristotle the (...) theoretician than into Aristotle the pedagogue. For this reason, the Great Ethics has distinct advantages as an introduction to Aristotelian ethical thinking: it is simpler and clearer in its argumentation, matters such as the intellectual virtues are made suitably secondary to the practical focus, the moral virtues come through with a pleasing directness, and the work's syllogistic formalism gives it a transparency and accessibility that the other Ethics typically lack. Arius' Epitome, which relies heavily on this work, helps confirm its value and authenticity. Because the Great Ethics is generally neglected by scholars, less has been done to clear up its obscurities or to expose its structure. But to ignore it is to lose another and more instructive way of approaching and appreciating Aristotle's teaching. The translation is prefaced by an analytic outline of the whole, and the several sections of it are prefaced by brief summaries. The commentary supplies fuller descriptions and analyses, sorting out puzzles, removing misunderstandings, and resolving doubts of meaning and intention. Glassary is a companion volume to Glas. It offers English readers fuller access to the masterwork of Jacques Derrida, the leading philosopher in France. Derrida is important for his investigations of language, philosophy, and writing. He has perforated the boundaries between academic disciplines, has demonstrated the theological underpinnings of apparently atheological philosophies, and has thrown into question traditional notions about the "ownership" of ideas. Glas exemplifies Derrida's methodology of reading and his central philosophical and literary concerns. The reader fascinated (...) by its complexities will appreciate the assistance of Glassary. Written by the chief translator of Glas, John P. Leavey, Jr., it includes an essay by Gregory Ulmer and a foreword by Jacques Derrida. The book provides all of the apparatus a reader of Glas might immediately desire, including notes on difficult or ambiguous passages, identifications of allusions and puns, locations of citations, and translations of passages in languages other than French. But Leavey does not stop there. He includes a glossary of use to readers of Glas in any language and essays that relate it to Derrida's texts and to the modern French critical enterprise as a whole. Oxford: Oxford University Press, 2005. viii + 433 pp. Cloth, $125.Due to their putatively lighter tone, exotic foreign settings, and concluding "resolutions" of past misfortunes, Euripides' Helen, fragmentary Andromeda, and Iphigenia Among the Taurians (henceforth IT) have often been described as (...) proto-romances or tragi-comedies. Wright's study follows recent trends in defining ancient Greek dramatic genres above all through their performance context; he argues that the plays belong firmly in the tragic camp and develop serious philosophical and religious themes that deliberately leave their audience in a state of aporia. The plots of these plays share an escape from disaster/remote and threatening environments. Although we cannot firmly date IT, Wright tries to make the case that these apparently unique shared elements suggest that the plays were performed as a coherent trilogy in 412 B.C.E. The book does not [End Page 465] attempt to address in detail many aspects of these (relatively neglected) plays that have been extensively discussed by other critics. Instead Wright focuses on the plays' generic status, their possible mythical innovations, their remote settings, and their philosophy and theology.Although there are many unusual mythical elements in these plays, Wright makes a detailed case that Euripides most probably recombined elements previously known in the tradition to a novel effect, rather than inventing them. He casts considerable doubt on Stesichorus' palinode and Herodotus as origins for Euripides' innocent Egyptian Helen, however. Wright challenges previous assumptions by arguing that Euripides does not develop the plays' barbarian settings. Indeed, there is little ethnic or geographical detail beyond a few initial sketchy references, and the plays blur the differences between Greeks and barbarian "others." The Egyptians Proteus, Theoclymenus, and Theonoe are, genealogically speaking, Greek (183). After a lengthy discussion of the problems of defining philosophical elements in tragedy, Wright links the plays' extensive treatment of the unreliable nature of human language and knowledge of reality to discussions of comparable themes in the fragments of the Sophists, especially Gorgias and Democritus. Finally, Wright argues that the plays, by stressing the impossibility of understanding the divine forces that govern the universe, represent an unusually bleak theological picture. The dei ex machina that conclude Helen and IT belatedly outline mysterious divine patterns governing the confusing dramatic events, but humans can only repeatedly misinterpret their experience and name the divine forces governing their lives, erroneously in Wright's view, as tuche\.These issues, above all the themes of illusion and reality and the nature of divinity in the plays, have in fact been discussed before, but Wright attempts a fuller (with the exception of his attenuated discussion of ritual) and methodologically tighter discussion. This represents both the book's strength and weakness. Without its exhaustive review of dated and (putatively) weakly argued earlier critical views, the book could, and in my view ideally should, have made its positive case more forcefully in half the length. At the same time, by failing to engage with important earlier treatments of central topics, Wright leaves the reader with the impression that scholarly work on these issues is far less nuanced than it has in fact been. To give a few examples that he neglects altogether, his discussion of Aristophanes' parody of Euripides in Thesmophoriazusae (120, n. 196; see also 50–51) does not take advantage of Zeitlin's seminal article on the topic ("Travesties of Gender and Genre in Aristophanes' Thesmophoriazusae," published most recently in her Playing the Other: Gender and Society in Classical Greek Literature [Chicago, 1996], 375–416); he does not make use of John H. Finley's extensive study of Euripides, Thucydides, and the sophists (Three Essays on Thucydides, [Cambridge, Mass., 1967]); other important methodologically sophisticated work on philosophy and tragedy, such as that of Mary Whitlock Blundell (Helping Friends and Harming Enemies: A Study in Sophocles and Greek Ethics [Cambridge, 1989]) or Christopher Gill (Personality in Greek Epic, Tragedy, and Philosophy: The Self in Dialogue [Oxford, 1996]), does not even appear in the notes. The bulk of this massive collection is comprised of selections from about twenty medieval, modern, and contemporary writers, on legal philosophy. These selections cover the traditions of natural law, positivism, and realism on the problem of the nature of law. It would be impossible to fault Professor Christie on the pieces he has included. Each one, old or new, is an acknowledged classic or standard. The omission of Lon L. Fuller who represents a notable variety of non-Thomistic natural law (...) should, however, be mentioned. In the very last part of the book, covering the topic of legal reasoning, the philosopher will be pleased to find extensive extracts from Aristotle and Francis Bacon. (shrink)

In this paper Popper formulates and discusses a new aspect of the theory of mind. This theory is partly based on his earlier developed interactionistic theory. It takes as its point of departure the observation that mind and physical forces have several properties in common, at least the following six: both are located, unextended, incorporeal, capable of acting on bodies, dependent upon body, capable of being influenced by bodies. Other properties such as intensity and extension in time may be added. (...) It is argued that a fuller understanding of the nature of forces is essential for the analysis of the mind-brain problem. The relative autonomy and indeterministic nature of mind is stressed. Indeterminism is treated in relation to a theorem of Hadamard. The computer theory of mind and the Turing test are criticized. William James once said that truth is “the good in the way of belief.” This has the ring of, well, truth. While it may appear as if James’ claim is straightforwardly true, I think that there are at least three different dimensions along which truth can be normatively related to belief. In this paper, I explore these different dimensions of truth’s value, considering both how they differ and how they relate. As we will see, our understanding of these different dimensions (...) of truth’s value can impact our understanding of its nature. I argue that a fuller understanding of the connections between truth’s normative dimensions pushes us in a decidedly pragmatist and pluralist direction—something of which I believe James would approve.
